Primary Care Physician (PCP) Role Overview

ESSENTIAL JOB DUTIES / RESPONSIBILITIES

  • Independently provide care for patients with acute and chronic illnesses encountered in older adult care.
  • Take full accountability for patient care and outcomes, seeking consultation from specialists when needed while remaining responsible for detailed care.
  • Engage with hospitalists for patients in the hospital, regardless of affiliation.
  • Carry out assessment, diagnosis, treatment, management, education, health promotion, and care coordination, and document for patients with acute and complex chronic needs.
  • Lead the care team consisting of a care promoter (medical assistant), care facilitator, and care coordinator for patients able to attend the office.
  • For patients unable to come to the office (in hospital, SNF, LTC, or homebound), engage with the transitional care team and other resources available in the market.
  • Actively participate in center management, cover for other providers when necessary, and contribute to recruiting patients and additional providers.
  • Collaborate with, supervise, or function within a practice or collaborative agreement with an Advanced Practice Practitioner (APP) and remain accountable for the APP’s actions.
  • Perform other duties as assigned and modified at the manager’s discretion.

  • Availability and accessibility for patients to build trust, including walk‑in visits and timely phone/messenger response.
  • Service orientation – provide care that the PCP would want for a family member or themselves to each patient at every interaction.
  • Evidence‑based medicine – stay updated on clinical, nutritional, and lifestyle‑based interventions to improve outcomes.
  • Physician leadership – continuously develop and improve leadership skills for the benefit of patients, teams, center, and company.
  • Quality focus – work with care teams to achieve high quality outcomes and continuously improve practice operations.
  • Influence – competently and compassionately influence patients and teams to achieve the best outcomes.
  • Self‑care – maintain physical, mental, social, and spiritual well‑being to provide optimal care.
  • Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook) and related software.
  • Willingness and ability to travel locally as needed, nationally for initial training (2 weeks) and occasionally regionally or nationally for recruiting or training.
  • Fluency in English, verbal and written. Fluency in additional languages may be required for some centers.
  • Independently exercise judgment in use of technology and clinical decision making.

PAY RANGE

$221,141 - $315,915 salary
