Hours of Work: Part-time, 26 Hours per Week
Pathway Coordinators are an integral part of the Outpatient team, looking after patients from the point of referral to discharge, ensuring the process is both efficient and effective.
Location: Outpatients department at Dorking Community Hospital.
Responsibilities
- Prepare clinic appointments for clinicians across all specialties, including expected appointments, routine results, pathology results for filing and patient queries.
- Review new patient referrals, picking out pre‑completed investigations and adding them to the electronic record.
- For follow‑up appointments, review prior clinic letters and referrals, ensuring investigations have been completed and results are available.
- Process post‑clinic folder documentation, distributing and actioning contents as required.
- Manage the shared “Pathways” email box and shared tasks on the patient record system, action them in a timely manner.
- Communicate and send results of in‑house diagnostic tests to the patient’s GP within specified timeframes.
- For external results, review, add to the record, and take necessary follow‑up actions, removing patients from wait‑lists and booking follow‑up appointments.
- Act immediately on urgent findings, following established processes to inform clinicians and progress patient care.
- Chase reports, results and other documentation from GPs, hospitals and third‑party providers, working proactively and independently to achieve required outcomes.
- Track patient pathways from referral to discharge, highlighting and resolving queries as encountered.
- Maintain excellent working relationships with colleagues, consultants and external providers.
- Maintain accurate patient records, updating wait‑lists, tasks, delivery/read receipts and adding results and notes.
- Manage multiple tasks, prioritising own workload and working with the team to meet target timeframes.
- Undertake any other reasonable duties and mandatory training as required.
Skills & Qualifications
- Excellent attention to detail and organisational skills.
- Strong communication skills with a diverse range of people.
- Ability to prioritise tasks and exercise good judgement.
- Self‑motivated, proactive team player with good interpersonal skills.
- Proven problem‑solving experience.
- Experience in a patient / customer‑focused role.
- Knowledge of IT systems and data protection in a healthcare environment.
- Understanding of medical terminology; experience working in a fast‑paced environment.
- Minimum good standard of education, numeracy and written/spoken English.
- Experience with NHS databases such as SystmOne (preferred).
