Pathway Coordinators are an integral part of the Outpatient team, looking after patients from the point of referral to discharge, ensuring that the process is both efficient and effective. This is achieved by accurately processing and actively monitoring the flow of information throughout the patient pathway. The role would suit someone who is good at multitasking, problem‑solving with an empathetic nature, who has a passion for helping patients.

Key Responsibilities

  • Ensuring that the patient journey is both efficient and effective.
  • Chasing reports, results, and other documentation with GPs, hospitals, and third‑party providers to ensure these are in place prior to appointments.
  • Highlighting any urgent findings to our clinicians to expedite patient care where required.
  • Managing the workload efficiently and effectively on a daily basis to facilitate the patient's care being as smooth and efficient as possible.
  • Maintaining patient confidentiality at all times.

Candidate Profile

  • Ability to remain calm and maintain a high level of attention to detail.
  • A can‑do attitude is a vital asset.
  • Good working knowledge of Microsoft Office would be beneficial.

The Pathways team is currently made up of 8 staff members based in the Outpatients department at Dorking Community Hospital. As we are seeing the number of patients grow, we are looking to expand our team and would be interested in people who could offer between 30 to 37.5 hrs a week.
