An exciting opportunity has arisen for a Practice Nurse to join the Nursing Team at Donnington Medical Practice, Telford, Shropshire. We are committed to the development of a happy, hardworking and highly effective health care team.

The successful applicant will join an enthusiastic team of nurses and HCAs and help to deliver excellent nursing care to our local population providing a range of Practice Nurse Duties including:

  • Chronic Disease Management (Diabetes/COPD/Asthma)
  • Experience of QOF
  • Childhood & Travel Immunisations
  • Cervical Smear Screening
  • Dressing & Wound Care
  • Contraception
  • ECGs
  • Health Promotion/Advice
  • Signposting to other appropriate services

Main duties of the job include:

  • Comprehensive understanding of Chronic disease management
  • Monitoring the health of patients with chronic disease and co-morbidities
  • Giving advice and support to patients to promote good self-management in line with practice protocol and local formulary
  • Contributing to the development of practice protocols and audits
  • Ensuring knowledge and skills are kept up-to-date in line with current guidelines
  • Participating in the provision of general practice nursing including Smears/Cytology, child immunisations, travel immunisations, wound care/ulcer, venepuncture
  • Assisting and performing routine tasks related to patient care as directed by senior nursing staff and GPs
  • Contributing to a happy, healthy work environment

About us: We are an established Practice with 4 GP partners, 5 salaried GPs and a Nursing team which includes an Advanced Nurse Practitioner, 2 Practice Nurses and 2 HCAs. All of us are supported by an excellent team of administrators, secretaries and care navigators. Located in the beautiful county of Shropshire, we are in close proximity to both countryside and excellent links to a comprehensive travel network.

Job responsibilities include assisting medical personnel in the care of practice patients, including treatment, preventative care, screening and patient education, and working with reference to practice agreed protocols and guidelines, providing nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.

Essential experience, knowledge and skills include:

  • Respiratory Management: Understanding of asthma and/or COPD care, maintaining up-to-date register of asthmatic and COPD patients, monitoring health of patients with respiratory disease, giving advice and support to promote good self-management
  • Diabetes Management: Understanding of Diabetes, maintaining up-to-date register of diabetic patients, monitoring health of diabetic patients against key indicators, liaising with other health professionals involved in the delivery of diabetic care
  • Chronic Disease Management: Understanding of Coronary Heart Disease, Cardiovascular Disease, Hypertension, Chronic Kidney Disease and Peripheral Arterial Disease; ensuring knowledge and skills base are kept up to date
  • Sexual Health: Advising patients on the National and Local Cervical Cytology Mammography screening programmes, undertaking accredited training and performing cervical cytology sampling under supervision until competent
