Job Summary

Due to rapid, continued growth, an exciting opportunity has arisen for an experienced Advanced Nurse Practitioner to join our practice in Donnington, Telford. This role involves working alongside a multidisciplinary team, including GPs, Nurse Practitioners and Clinical Pharmacists, to provide care and treatment to patients with long‑term conditions or undiagnosed undifferentiated diagnoses.

Job Responsibilities

  • Work as an autonomous practitioner, diagnosing, managing, prescribing, and discharging patients across all age groups, including young children, via face‑to‑face, telephone or video consultation.
  • Provide clinical nursing practice at an advanced level, delivering holistic care to people accessing primary health care services.
  • Assess diagnoses and treat patients with acute, non‑acute and chronic medical conditions.
  • Assess and treat patients in surgery or their own home who require acute medical attention and refer to appropriate agency.
  • Admit patients directly to secondary care hospitals when acute medical need arises.
  • Refer patients to other medical specialities or members of the multidisciplinary team, to secondary care or statutory and voluntary organisations, including referral for X‑ray.
  • Interpret a range of diagnostic tests and routine clinical procedures.
  • Make critical judgments, including first contact with acutely presenting patients, with or without existing protocols.
  • Make decisions where precedents do not exist and refer directly to hospital consultants without prior reference to other medical colleagues.
  • Advise and support others where standard protocols do not apply.
  • Operate within professional guidelines, codes of conduct and all relevant practice policies, including infection control, chaperoning and risk management.
  • Serve as an extended and supplementary prescriber.

Person Specification

Essential Qualifications

  • Active registration with the Nursing and Midwifery Council (NMC).
  • Practice within the four pillars.
  • Educated to Masters level.
  • Independent prescriber.
  • Meet NMC revalidation requirements.
  • Disclosure and Barring Service check completed.
  • UK professional registration current.

Desirable Qualifications

  • QOF Lead experience.
