Key Responsibilities:

  • Managing and updating rotas to meet client needs.
  • Supporting care staff, providing guidance and operational oversight.
  • Handling on-call duties to maintain continuity of care.
  • Liaising with clients, families and professionals to ensure high standards of care and satisfaction.
  • Responding to changes in care requirements, including hospital discharges and urgent staffing needs.
  • Monitoring compliance, care plans and service delivery in line with company and regulatory standards.
  • Maintaining accurate records and utilising care systems effectively to support service delivery.

Essential Requirements:

  • Previous experience as a Care Coordinator within domiciliary care, live-in care or supported living.
  • Experience completing on-call duties and managing out-of-hours issues.
  • Strong organisational skills with the ability to prioritise in a fast-paced environment.
  • Positive and personable, able to work effectively within a busy, open office environment
  • Confidence using digital care planning and rostering systems.
  • Strong communication skills, able to build relationships with staff, clients and management.
