We are seeking a Senior Care & Support Worker on a part‐time basis to cover maternity leave, working 2 shifts per week. To participate as part of the Care Team in providing care to a chronically ill client with complex needs requiring a rehabilitative programme. The post holder will be part of a team providing care, in shift patterns, to an individual client within a Care Home setting.

Duties:

  • Ensure that the highest possible standard of care is maintained within a safe environment using person‐centred practice.
  • Provide safe and effective care including all aspects of personal and specialist care such as tracheostomy and suctioning management.
  • Provide all aspects of care according to the client care plan.
  • Follow agreed systems and protocols.
  • Liaise with client, families, care management and other Trust representatives as required.
  • Ensure proper use, care and maintenance of nursing aids and equipment including those for moving and handling purposes.
  • Efficient use of continence aids in line with guidelines and liaising with Continence Service as necessary.
  • Assist clients to maintain optimal function.
  • Obtain client consent for all procedures.
  • Implement agreed systems to ensure adherence to guidelines and MD Healthcare Policy for the safe handling, administration, storage and custody of medicinal products.
  • Adhere to infection control policies and procedures.
  • Maintain respect for client property and privacy.
  • Attend client reviews and meetings as required.
  • Practice in a way that acknowledges the dignity, cultures, values, beliefs and rights of the client.
  • Adhere to all NISSC Codes, Standards and Guidelines and maintain up to date NISCC registration.

Communication:

  • Act on any changes in the client's general condition as per protocols.
  • Follow incident reporting procedures.
  • Participate in implementation of record keeping in accordance with guidelines and adhere to MD Healthcare policy on records management.
  • Attend and participate in staff meetings as required.
  • Able to work autonomously and as part of a team and maintain confidentiality at all times.

Personal and People Development:

  • Maintain own personal and professional development in accordance with NISCC Code of Conduct.
  • Attend mandatory training and the necessary training arranged by MD Healthcare to enable competent provision of all aspects of clinical care according to the client care plan.
  • Assist with the induction, orientation and training of new staff.
  • Participate in the organisation's supervision programme.

General Responsibilities:

  • Carry out duties and responsibilities in a manner which assures client and carer safety.
  • Demonstrate commitment by regular attendance and efficient completion of all tasks allocated.
  • Comply with the company No Smoking Policy.
  • Carry out duties and responsibilities in compliance with health and safety policy and statutory regulations.

  • Flexible approach to working hours to ensure smooth and continual running of Complex Needs Service.

Qualifications:
