PCN Pharmacist in Ware

We cover about 37,000 patients across two practices in Hertfordshire — Dolphin House Surgery in Ware and The Buntingford & Puckeridge Medical Practice. Between them, the sites serve a mix of market-town and semi-rural communities, and both are long-established training practices. Our pharmacy team is one we’re genuinely proud of. There’s a Senior Clinical Pharmacist, three Clinical Pharmacists, and a Pharmacy Technician — all experienced, all collaborative, and all used to working as a proper team rather than a collection of individuals sharing a rota. We’re adding to that team now, and we’d like the person who joins to feel at home in it.

The role

This is a clinical pharmacist post under the ARRS, offered anywhere from part-time up to full-time. You’ll work across just two sites, allowing you to build real relationships and continuity with patients and colleagues — rather than constantly moving between practices. Independent prescribing is desirable but not a barrier to applying. If you’re not yet qualified, we’ll put proper support in place to get you there — as training practices, we have a track record of actually doing this rather than just saying we will. We’re also open to someone who wants to develop a specialist interest over time — care homes, diabetes, respiratory, medicines optimisation, or something else that suits both you and the population we serve. That’s a conversation we’re willing to have seriously, not just something to put in an advert.

What you’ll be doing

• Structured Medication Reviews (SMRs), particularly for care home patients and those with complex or multiple long-term conditions
• High-risk drug monitoring and medicines safety, including shared care protocols
• Long-term condition clinics — diabetes, hypertension, asthma/COPD — alongside the wider clinical team
• Repeat prescriptions and medication queries
• Medicines optimisation across both practices: prescribing audits, quality improvement work
• Contributing to PCN DES requirements and IIF targets through pharmacist-led activity
• Working with GPs, nurses, care co-ordinators, and other ARRS colleagues day to day
• Mentoring junior pharmacy team members, including our pre-registration trainee

What we need

• GPhC registration
• MPharm degree or equivalent
• Completion of, or active enrolment in, the CPPE Primary Care Pharmacy Education Pathway (or equivalent) — or willingness to start
• You actually want to work with patients, not just manage their medication from a desk
• Good communication skills and the ability to work well in a multidisciplinary team
• You keep your CPD current
• Independent prescribing qualification (or willingness to work towards it with our support)
• Previous experience in general practice or a PCN setting
• Experience of SMRs or medication reviews in a clinical context
• Interest in a specialist area such as care homes, long-term conditions, or medicines safety

This is a role where you can settle, develop, and make a visible difference. You won’t be stretched across multiple sites or expected to hit the ground running without proper support. Instead, you’ll be part of a team that values continuity, collaboration, and doing the job properly. We’re also conscious that the best pharmacists have options right now — community, hospital, other PCNs. We’re not going to pretend otherwise.
