Disability Solutions West Midlands is seeking an Equity Support Worker to deliver its Life Without Limits project, supporting young people with disabilities aged 16 to 25 across Stoke-on-Trent as they prepare for independent living.
You will provide person-centred enablement support, one-to-one mentoring, co-produced action planning, workshops and group activities, and support moves from family homes or local authority care into independent living.
