Key Job Details

  • Salary: £26,000 per annum
  • Mileage paid at 45p/mile
  • Reports to: Central Cornwall Operations Manager
  • Contract: Permanent subject to funding
  • Holiday: 31-43 days
  • Location: Community based Central Cornwall - candidates will ideally be located in or close to either Truro or Falmouth with potential travel required to Newquay
  • Hours: 37 per week, Monday to Thursday 9am to 5pm, and Friday 9am to 4.30pm

Role Purpose

To provide person-centred care and support to people across central Cornwall, enabling them to live independently and recover confidence at home and in the community. This role focuses on holistic support, helping clients address both immediate needs and longer-term challenges to enhance their wellbeing. This role does not provide personal care.

Key Responsibilities

  • Conduct home visits post-hospital discharge or for those identified as needing support, ensuring safety, comfort, and essential needs are met (e.g., meals, heating, medication)
  • Perform holistic assessments and develop action plans covering housing, welfare benefits, adaptations, and other support needs
  • Communicate with internal teams (e.g., Support Broker, Advice Team) and other agencies to ensure clients’ needs are addressed
  • Provide temporary practical assistance (e.g., shopping, meal prep, light cleaning) until permanent arrangements are in place
  • Ensure clients have access to emergency contacts and ongoing support arrangements
  • Address any issues impacting a client’s recovery or wellbeing, escalating concerns when necessary

Eligibility and Skills

  • Eligibility to work in the UK and access to a vehicle with business insurance
  • Communication, organisational, and IT skills
  • Ability to work independently, multitask, and adapt to changing situations
  • Familiarity with care assessments, risk assessments, and local support services
  • Experience using case management systems and supporting individuals post-discharge
  • Experience in delivering support services
  • Knowledge of health, social care, or community support roles
  • Knowledge of local resources and networks
  • Familiarity with environmental assessments and safety standards
