A 89 year old gentleman who resides in Burry Port
The individual will access his community as per his wishes
The Individual will be supported to engage in his community activities of choice.
HOW OUTCOMES WILL BE ACHIEVED:
- The Individual and his PA to arrange times and days of choice.
- PA to ensure the Individual is taken out around his care support to ensure medication and nutritional needs are met.
- PA to ensure the Individual has his wheelchair, at times to be folded and input into car if they go out further than his local community .
- PA to accompany the Individual to places of interest to him.
