Responsibilities:
- Managing a team of colleagues to help people gain more independence in their daily lives.
- Developing and implementing person‑centred support plans.
- Conducting care needs assessments and completing medication competencies and audits.
- Recruiting and inducting new colleagues, completing supervisions/appraisals.
- Attending multi‑disciplinary meetings, completing reports, and assisting the Registered Manager with day‑to‑day management.
- Participation in the on‑call rota, as required.
Qualifications and Experience:
- Previous experience in a care management role.
- Level 3 qualification in Health & Social Care.
- Experience working with individuals with autism, acquired brain injury, complex mental health needs, traumatic backgrounds and challenging behaviours.
