An exciting opportunity has arisen for an experienced Practice Nurse to join our friendly team. The position is for full time 37.5hrs per week.

Main duties of the job:

  • Delivering nursing services including immunisations, vaccinations, cervical cytology, and health promotion.
  • Support the management of long‑term conditions such as diabetes, asthma, COPD, and hypertension.
  • Undertake chronic disease monitoring and patient education.
  • Administer medication and vaccinations in accordance with practice protocols.
  • Promote health education and wellbeing initiatives.
  • Work collaboratively with GPs, healthcare assistants, pharmacists, and administrative staff.
  • Participate in infection prevention and control procedures.
  • Support quality improvement and clinical governance activities.

Clinical responsibilities:

  • Provide high‑quality care for acute and long‑term conditions.
  • Maintain accurate, comprehensive records and documentation.
  • Prescribe within clinical competence if qualified.
  • Deliver care in accordance with local and national guidelines.
  • Undertake procedures including but not limited to cervical cytology, immunisation and vaccination, long‑term conditions management, venepuncture, BP and spirometry monitoring, health screening and lifestyle education.

Administration responsibilities:

  • Contribute to Quality and Outcomes Framework (QOF) targets.
  • Participate in audits and quality improvement initiatives.
  • Maintain clinical equipment, cold chain compliance, and stock levels.
  • Ensure adherence to clinical governance and infection control policies.
  • Assist with protocol development and service improvement, supporting practice performance targets and contractual compliance.

Professional responsibilities:

  • Maintain NMC registration and CPD.
  • Adhere to all mandatory training requirements.
  • Work within the NMC Code and NHS guidelines.
  • Promote equality, diversity, and inclusion.
  • Handle sensitive information with discretion and in line with GDPR.

Qualifications:

  • Registered Nurse (Nursing and Midwifery Council).
  • Current NMC registration.

Experience:

  • Experience of working as a Practice Nurse.
  • Experience of chronic disease management.
  • Experience of working autonomously.
  • Experience of working in Primary Care.

Knowledge and skills:

  • Excellent communication skills – written and oral.
  • Effective time management – planning and organising.
  • Ability to work as part of a team member and autonomously.
  • Good interpersonal skills.
  • Problem solving and analytical skills.
  • Ability to follow clinical policy and procedure.
  • Ability to communicate complex and sensitive information effectively over telephone, email and face to face.
  • Request pathology tests and process the results, advising patients accordingly.
  • Understanding of evidence‑based practice.
  • Ability to promote best practice regarding nursing matters.
  • Ability to work within own scope of practice and understand when to refer to GPs.
  • Knowledge of SystmOne.
  • Knowledge of public health issues in the local area.

About us:

We are a friendly and forward‑thinking GP practice committed to delivering high‑quality patient‑centred care to our local community. We are looking to recruit a motivated and compassionate Practice Nurse with experience of working in general practice to join our team. The partnership consists of two GPs who are supported by a salaried GP, one ACP, one practice nurse, one HCA and a fantastic administration team. We also host medical students. Our current list size is approx 9,000 patients. We use System One clinical system. Our aim is to provide the best possible patient care to our patients in a safe and welcoming environment. We are unable to offer a Working Visa for this position.
