About the Job:
A rare opportunity has arisen for an enthusiastic and motivated either full time or part time Band 3 health care assistant to join our team, for posts in either Doddington or Huntingdon. The main aspect of the role is to care for patients with chronic respiratory disease in the home setting and in our Pulmonary Rehabilitation Classes.
The post entails supporting the health care professionals in providing expert care for respiratory patients, by helping to prevent avoidable hospital attendances, providing facilitated support following hospital admission, assisting in Pulmonary Rehabilitation Classes and the promotion of focused self management. The successful candidate will be required to work flexibly across the countywide respiratory service to cover, as necessary.
We are a very supportive and friendly team. It is anticipated that the successful candidate will have respiratory and/or community experience, however full training and support is available.
Main duties of the job
To work as a member of the Community Respiratory Team, alongside other support workers, nurses, and physiotherapists. With training, use skills from the professional areas of Nursing, Allied Health Professionals and Medicines Management to undertake tasks as delegated to meet the individual needs of service users. This could include inhaler technique reviews, home exercise programmes or ambulatory oxygen reviews.
To support the physiotherapist in the delivery of face to face and virtual pulmonary rehabilitation programmes. Motivating patients and supporting them throughout the course. To assist in the delivery of high standards of health care and support patients within their community setting by following prescribed packages of care. Providing care to patients with deterioration in their health or function with a particular focus on admission avoidance to acute hospitals.
To deliver functional assessments, including mobility and appropriate exercise and equipment provision To support staff in the management and execution of clinical practice To carry out duties delegated and supervised by registered professionals. To provide a high standard of care To manage own caseload of patients who require ongoing management and support with complex needs.
