About the Job:

Main duties of the job

The post holder must be an experienced Healthcare Assistant in general practice, who can support in managing specific cohorts of patients with a range of acute and chronic medical conditions. They will work as part of the team, delivering care within their scope of practice to patients within the surgery and conducting home visits. They will deliver and assist clinical staff in the provision of treatment, preventative care, health promotion and patient education. They will carry out baseline observations such as pulse oximetry, blood pressure, phlebotomy, BMI, temperature as directed and record findings accurately. They will support with management of chronic disease, delivering opportunistic health promotion where appropriate. The post holder may complete basic care plans.

About us

The role will support four practices within the Glossop area, with approx. 33,350 patients. This role will work alongside PCN Additional Roles practice including social prescribing link workers, mental health workers, paramedics and pharmacists amongst others.

We are looking for a caring and reliable Healthcare Assistant to join our community care team. This role involves supporting patients in their own homes and community settings, delivering high-quality care and promoting independence. You will work closely with our community matron, GPs, and other health professionals to ensure patients receive safe, compassionate, and person-centred care.

Essential

  • Good communication skills (written & verbal)
  • Good organisational skills
  • Good customer service skills
  • Ability to work flexibly to meet the demands of the service

Desirable

  • Demonstrate an interest or previous experience in caring for patients in an acute setting
  • Basic knowledge of IT packages
  • Experience of working in a team

Qualifications

Essential

  • Level 1 literacy and numeracy (GCSE English and Maths Grade D-G)

Desirable

  • NVQ level 3 in Healthcare

Skills

Essential

  • Previous experience of working in a primary care/general practice setting.

  • Use of EMIS
