We are looking for a dynamic, motivated and enthusiastic person who has excellent communication, IT literate and problem solving skills alongside a passion for patient centered care.
Main duties of the job
You will be part of an MDT team in which you will receive clinical and managerial support, which is also part of a wider MDT team that is aligned to social care professionals in reflection of the NHS 10 year plan.

The successful candidate will be expected to be flexible and assist colleagues to cover service needs, which will include working across the wider integrated team. Access to a car in order to carry out service duties is essential.

The Community Nursing Service operates between 08.00 to 21.00 hours and the working pattern will include a variety of rotational shifts which will include weekend, evening and Bank Holiday working.
