We have an exciting opportunity for a passionate and experienced Registered Nurse to join our team as a Registered Nurse Liaison. A role that blends clinical skill with meaningful human connection.
Are you a compassionate, confident Registered Nurse who loves supporting residents and families through one of the most important transitions of their lives? We’re looking for someone who blends clinical expertise with warmth, clarity, and exceptional communication. This role is perfect for a nurse who enjoys autonomy, relationship-building, and ensuring every new resident feels safe, understood, and welcomed from the very first conversation.
Responsibilities- Leading timely, person-centred clinical pre-assessments in hospitals, homes, or community settings.
- Managing referrals with professionalism, accuracy, and excellent communication.
- Coordinating smooth, safe admissions with our care, hospitality, and housekeeping teams.
- Providing daily first-week check-ins to support residents and families as they settle.
- Acting as a consistent, reassuring point of contact during the transition into the home.
- Ensuring all documentation is completed to a high standard.
- Promoting dignity, respect, and psychological safety in every interaction.

Qualifications- NMC-registered Nurse (Adult or Mental Health).
- Strong clinical assessment and decision-making skills.
- Warm, confident communication with residents, families, and MDT partners.
- Ability to manage sensitive conversations with empathy and clarity.
- Organised, proactive, and calm under pressure.
- Experience in older adult care or dementia care is an advantage.
