Make everyday support feel extraordinary. MSH Healthcare Ltd is looking for a Care Liaison Officer to join our friendly, community-based domiciliary care team. This key role bridges communication between service-users, families, care teams, and professionals—ensuring smooth onboarding, responsive coordination, and high-quality support.

You will also have a dedicated team of carers under your wing—supporting their day-to-day needs, keeping them informed, and helping them deliver compassionate, person-centred care.

Your mission:

  • Be the first point of contact for new clients and families—explaining services, booking assessments, and handling initial paperwork.
  • Build strong relationships with your team of carers—supporting them with daily queries, care updates, and rota coordination.
  • Coordinate feedback from GPs, nurses, OTs and social workers to help develop and update person-centred care plans.
  • Maintain regular contact with service-users and relatives, providing updates and resolving concerns.
  • Carry out satisfaction calls/visits after onboarding and every six weeks—recording and escalating feedback where needed.
  • Work closely with schedulers to match carers’ skills and availability with client needs, including rota problem-solving.
  • Identify and elevate any risks or safeguarding concerns to the appropriate leads within two hours.
  • Respond to stage-one complaints within set timeframes—investigating and resolving where possible.
  • Represent MSH Healthcare Ltd at discharge planning meetings, community events, and local forums to build partnerships.
  • Keep accurate, GDPR-compliant notes using our electronic care management system.
  • Filling in on care for new packages and mentoring in new recruits.

Experience:

  • Experience in health or social care, ideally within home care or care coordination roles.
  • Excellent communication and interpersonal skills.
  • Organised and proactive approach to multi-tasking.
  • Comfortable using digital care systems and handling sensitive data.
  • Understanding of safeguarding, GDPR, and CQC expectations.
  • Full UK driving licence and access to a car (essential).
