Description We are currently recruiting a Social Worker – Level 2/3 for Hospital Discharges. To provide assessment, care management and social work services to users and carers arising out of the duties and powers under legislation, and Group Policy.  To work following the guidelines set out by local management and to make appropriate use of group and independent resources. Responsibility To assess the needs of People who are in hospital who may require Community Care Services, on request, and to provide Care Plans and arrange for the provision of services where the Eligibility Criteria are met. Qualification – Essential GCSE Grade (or equivalent) in English Language and Mathematics. DipSW Qualified or Equivalent HCPC Registration / GSCC Registered Proof of Qualifications Advanced – Microsoft Office (Word, Excel, Outlook), social media Qualifications, Knowledge, and Experience Requirements – Criteria to be Evaluated at the Shortlisting Stage.  Successful candidates will have: The ability to be assertive and challenging to safeguard and promote the welfare of children. Knowledge of key legislation and responsibilities of the local authority, including the NHS and Community Care Act 1990, the Community Care (Delayed Discharge) Act 2003 and the Mental Capacity Act 2005. The ability to communicate with staff at all levels. Experience and ability to deal with sensitive issues. Ability to manage workload and work as part of a team. Demonstrate the ability to meet tight deadlines. Experience working as a social worker in a hospital setting. Experience and ability to respond to a crisis. The ability to make sound assessments within a multi-disciplinary context of the needs of vulnerable people, produce and implement appropriate care plans and maintain accurate reports. The ability to supervise others. Ability to work as part of a small team and on own initiative, Proven analytical skills Advanced computer literate. Able to show empathy, discretion, and the ability to keep confidences, be trustworthy and reliable To have excellent listening skills. Knowledge of the Children Act regarding guidance and regulations of looked-after children. Able to maintain high personal and professional standards, to take a proactive approach to equal opportunities, and to achieve the highest possible quality of service in partnership with service users, carers, and other agencies. Essential Compliance Requirements 5 Years References Enhanced CRB/DBS Within Last 12 Months A pre-engagement screening is mandatory for this role.
