We seek a dynamic Personal Advisor for the Looked After Leaving Care Service. This role is significant as it is designed to support care leavers successfully transitioning to adulthood. Your contribution to the assessments, formulation, implementation, and review of Pathway Plans through direct interventions and full engagement of the young person will have a profound impact.

Responsibilities:

  • Provide coaching and motivational support to enable young people to build the confidence and self-esteem to be successful in adulthood.
  • Safeguard and protect care leavers as they transition to independence through regular contact.

Qualifications – Essential:

  • GCSE Grade (or equivalent) in English Language and Mathematics.
  • Degree level or extensive experience directly working with children and young people.
  • Advanced skills in Microsoft Office (Word, Excel, Outlook), social media and CRM.

Qualifications, Knowledge, and Experience Requirements:

  • Experience providing personal advisors for eligible, relevant and former relevant young people, as required by the Leaving Care Act 2000 and volume 3 of the Children Act 2010.
  • Experience in producing and reviewing the pathway plan and regular 2-monthly visits to care leavers.
  • Able to help young people contribute to assessing their needs before their transfer to the Leaving Care Team.
  • Able to review and discuss the pathway plan for 16/17-year-olds with CLAFH colleagues.
  • Able to assess young people's financial needs and organise payments, claim benefits or grants from education or charitable funds, and, where necessary, support young people in spending their leave care grants.
  • Able to maintain appropriate statutory and administrative records, including using IT systems.
  • Able to maintain confidentiality and observe data protection guidelines.
  • Experience assisting young people and their social workers when they reach 16 in their preparation for leaving care by providing advice and a range of group work sessions to young people.
  • Experience developing independent living skills, educational opportunities and identifying accommodation options.
  • Able to support young people once they reach 18 by providing a range of individual and group work sessions designed to improve their life skills, relieve social isolation and enhance peer group support.
  • Knowledge of accommodation providers and working with young people to ensure contact and security for the license or tenancy.
  • Able to develop and maintain positive links with agencies such as Connexions, careers, training agencies, colleges, local employers and the community to promote young people's access to employment, training, education and leisure activities.
  • The ability to involve young people in the planning, delivering, and evaluating services and encourage young people's involvement in the Children in Care.
  • Ability to work collaboratively with other professionals, families, and community members to provide or access support and achieve change.
  • Ability to assess needs, formulate and communicate plans of support and transitions to independence.
  • Ability to analyse information and implement SMART plans of action.
  • Ability to prepare simple reports and correspondence.
