Could you work in a busy and dynamic team, supporting individuals over the age of 65 with both organic and functional illness? The South Derbyshire and Dales Older Adult Community Mental Health Team (CMHT) are looking for an enthusiastic Lead Nurse who has a specific interest and passion in caring for older adults with Organic and Functional Illness. Ability to travel is essential.
Main Duties of the Job:
- Supporting and managing a complex caseload, performing initial assessments, assessing risk, developing collaborative care plans, and undertaking the role for duty worker.
- Administering depots, attending CST meetings and working closely with partners in health and social care.
- Supporting and developing Student Nurses and supervising junior members of the team.
- Duty working is also required as part of the role.
The successful candidates will receive regular clinical and managerial supervision and access to professional development. In the Older Peopleβs Division knowledge and skills development is actively encouraged, with training opportunities supported wherever possible.

Person Specification:
Essential Skills:
- Understanding team dynamics and promoting a positive working culture to support team cohesion.
- Ability to build effective working relationships internally and externally.
- Ability to work autonomously and set own priorities.
- Computer literate.
- Car driver / ability to meet travel requirements of the post.
Essential Knowledge and Experience:
- Experience working with people who have severe and enduring needs.
- Experience supervising staff and students.
- Detailed knowledge of legislation relevant to Mental Health and Specialist practice where appropriate.
- Working knowledge of the principles of Care Programme Approach where appropriate.
- Extensive knowledge of the principles of Safety planning.
- Experience post-qualification working with older adult patients who have an organic and/or functional illness.
- Knowledge of patient centred care planning.
Desirable:
- Post qualification community experience.

Attainment / Qualifications:
- Registered RN (RMN)
- Degree / Diploma in Nursing
