Provide compassionate, person-centred care to individuals at end of life in their own homes.
Support with personal care, comfort, nutrition, hydration, and mobility in line with care plans.
Offer emotional reassurance to patients, families, and carers during a sensitive time.
Observe and report changes in condition to the clinical team to ensure safe, responsive care.
