At DHU Primary Care, our teams provide high-quality, compassionate primary care to patients across our communities, often when they need us most.
Working across GP practices, primary care services and the wider community, our skilled clinical and support teams collaborate to deliver safe, responsive and personalised care.
As a Care Navigator you will
- Patient-focused reception: Provide a friendly, courteous and professional service to patients both face-to-face and by telephone, managing appointments, queries and signposting appropriately.
- Clinical administration: Process messages, emails, test results, repeat prescriptions and other administrative tasks accurately and promptly, following practice protocols.
- Practice operations: Support the smooth day-to-day running of both surgeries, including opening/closing, maintaining tidy reception and waiting areas, managing post and handling patient payments.
- Teamwork and flexibility: Work collaboratively with the wider practice team, attend training and meetings, and provide flexible cover for planned and unplanned staff absences.
- Professional standards and mobility: Follow all practice, health and safety and confidentiality procedures, maintain a professional appearance, and travel between Eyam and Bradwell Surgery as required.
