Our Urgent and Emergency Care teams deliver expert, compassionate support 24/7, across Emergency Departments, Urgent Treatment Centres, GP surgeries, and the wider community. Skilled medical, nursing, allied health, and support professionals work together to respond quickly and effectively, while you'll be supported to thrive in your career.

Provide hospital-level care to acutely ill patients within the Community Virtual Ward, supporting patients to remain safe at home and helping to prevent unnecessary hospital admissions. Responsible for patient assessments and clinical triage in line with SOPs and NICE guidelines, while working collaboratively with multidisciplinary teams to deliver high-quality, patient-centred care.

As a Community Virtual Ward Assessment Practitioner, you will:

  • Assess patients' clinical needs in relation to digital health and coordinate appropriate care and community support services.
  • Conduct telephone triage for identified health deterioration and emergencies, and escalate concerns appropriately using clinical judgement and early warning scores.
  • Deliver specialist patient assessments, care planning, implementation, and evaluation while maintaining accurate and timely clinical records in line with NMC/HCPC guidelines.
  • Provide reassurance, advice, and support to patients, carers, relatives, and colleagues, promoting effective communication and patient-centred care.
  • Work collaboratively within multidisciplinary teams to facilitate safe care pathways, early hospital discharge, and effective use of community services.
