As a Clinical Pharmacist you will deliver expert clinical medicines management, including structured medication reviews, medicines reconciliation, and support for patients with complex polypharmacy.
Responsibilities
- Deliver expert clinical medicines management, including structured medication reviews, medicines reconciliation, and support for patients with complex polypharmacy.
- Work as part of a multidisciplinary team across the practice and Primary Care Network to manage long‑term conditions, improve health outcomes, and reduce health inequalities.
- Lead on safe, effective prescribing systems, including repeat prescription processes and medicines optimisation across transitions of care.
- Contribute to quality improvement, clinical audit, and the Quality and Outcomes Framework, driving service efficiency and high standards of patient care.
- Provide specialist pharmaceutical advice, education, and independent prescribing support, promoting best practice and alignment with local and national healthcare priorities.
