DHU Healthcare is seeking a Care Navigator to join our Primary Care teams, providing essential front-line support to patients and clinicians across our Derbyshire practices.

Location: Eyam & Bradwell Surgeries, S32 5QH (with flexibility across Derbyshire practices)
Contract: 12-month fixed term contract
Hours: 0 hours
Competitive Salary: £12.21 per hour (Band 2)

What Your Typical Day Looks Like:
- Acting as the first point of contact for patients, providing a warm and professional welcome
- Navigating patients to the right care quickly and efficiently – whether that\’s a GP, nurse, pharmacist, or other services
- Managing appointment bookings and practice communications with accuracy and care
- Supporting clinical teams by handling essential admin and digital systems
- Providing clear information and signposting to local health and community services
- Contributing to practice efficiency, patient satisfaction, and quality improvement initiatives
- Working collaboratively with GPs, nurses, and other healthcare professionals
To Be a Successful Care Navigator You\’ll Need:
- Excellent communication and interpersonal skills
