DGS Health Ltd invites applications for a part-time Registered Nurse to support prevention, early intervention and physical health programmes across GP practices and community settings. You will deliver evidence-based nursing care, health promotion and preventative interventions as part of the Federation's Prevention and Physical Health Programme, including AF case finding, annual SMI physical health checks, cardiovascular risk assessments and MECC.
