Key Responsibilities

• Coordinate and deliver wellbeing-focused programs that support patients in managing their health and daily living.
• Plan, organize, and facilitate group sessions, workshops, and structured activities.
• Assess patient needs (physical, emotional, and psychosocial) and contribute to individualized care planning.
• Collaborate with multidisciplinary healthcare teams to ensure integrated patient support.
• Provide education and guidance that promotes self-care and independence.
• Support both in-person and remote/virtual patient engagement initiatives.
• Maintain accurate documentation and uphold professional clinical standards.

Requirements & Qualifications

• Registered Nurse qualification with valid professional registration (or equivalent allied health qualification).
• Relevant clinical experience in palliative care, community care, or related healthcare settings.
• Strong communication, interpersonal, and teamwork skills.
• Ability to assess patient needs and deliver person-centered care.
• Experience in teaching, mentoring, or facilitating learning is an advantage.
• Willingness to engage in continuous professional development or postgraduate study.
• Flexibility to support service needs, including occasional travel within the service area.
