The role

This company is pioneering value-based family building, bundling complex fertility treatments into fixed packages and absorbing the financial risk itself so patients do not pay in full without a successful outcome. Backed by leading global venture firms and partnering with hundreds of clinics across the US and UK, they operate an intricate multi-stakeholder ecosystem spanning patients, clinical networks, lenders, and insurers.

You will join as a Senior Product Engineer owning critical slices of the core orchestration platform, the real-time clinical underwriting engine, and production LLM agents that execute autonomous operational workflows. This is not a role building cosmetic frontends or tweaking isolated CRUD endpoints; it is a backend-heavy product engineering seat where you architect resilient distributed workflows across healthcare and fintech rails under strict data constraints.

The work

• Architect and scale the core orchestration framework, designing state machines that automate care navigation, financial underwriting, and clinic operations across disparate external systems.
• Build and harden the real-time underwriting engine that models medical outcomes and accurately prices financial risk.
• Implement and deploy autonomous, LLM-powered operational agents that execute production workflows rather than passive chat interfaces.
• Bridge legacy clinical records, lending infrastructure, and insurance rails under strict HIPAA and SOC 2 compliance standards.
• Take ambiguous business and clinical requirements from concept to production, maintaining high velocity without compromising system reliability.

What You Bring

• At least 5 years of full-stack or backend-focused engineering experience in fast-moving startup environments, ideally with significant tenure taking products from zero to scale.
• Strong Python proficiency, with hands-on experience architecting complex state machines, asynchronous workflows, and integrations between incompatible external APIs.
• AI-augmented engineering is already central to your daily workflow—tools like Cursor, Claude Code, or Codex actively accelerate how you think and ship.
• Demonstrated experience engineering around strict safety, compliance, or regulatory boundaries, such as HIPAA, SOC 2, or PCI.
• A pragmatic bias for shipping: you choose simple, resilient architectures over clever over-engineering and can break down ambiguous product problems independently.
