Our team includes experienced Band 5 and 6 OTs, Social Care Assessors, and OT apprentices all dedicated to supporting one another and sharing knowledge.

Main duties of the job

The Role

As a Community Occupational Therapist within Care Direct Plus, you will be a valued member of the team, delivering person-centered assessments and interventions within the framework of social care and health legislation.

You will manage your own caseload and work autonomously across a variety of settings, using your specialist clinical skills including moving and handling expertise to support individuals to live safely, independently, and with dignity.

Key Responsibilities

• Carry out specialist moving and handling assessments and reviews in both community and hospital settings especially where double-handed care is provided or requested.
• Develop and implement effective, person centered solutions that promote independence, including specialist equipment and tailored care packages.
• Manage a diverse caseload, including highly complex cases, in line with relevant legislation and best practice.
• Work as an autonomous practitioner, demonstrating strong problem solving, clinical reasoning, and independent decision making skills.
• Explore innovative, collaborative ways of working across general practice, acute and community health, social care, and voluntary and private sector partners.
