- Summary of job
The post holder will be responsible for the day to day clinical and operational service delivery to ensure that an individualised thorough evaluation and Care Management service is effectively given by the Duty Team.
This will entail clinical and operational supervision of junior nursing staff and students as authorised by the team manager.
To be responsible for delivering quality services that are effective and efficient against the organisation’s objectives and performance measures.
To collaborate with other agencies to support strategic service development.
Key responsibilities
- To work each day with the Duty Manager as the trained Duty Worker.
- To do comprehensive health needs and risk assessments (including capacity assessments) as well as build a care plan (Health Action Plan) in accordance with identified needs developing specialist course of responsibilities/actions using Recovery Principles.
- To do collaborative evaluation with other MDT as required.
- Always be aware of and comply with relevant mental health and work related legislation and policy.
- To give appropriate up to date evidence based information regarding treatment/intervention which enables service users to make informed choices in relation to their care.
- To provide evidence based care in a timely manner.
- To provide reactive recommendations when working alone in the community as appropriate.
- To engage in health promotion initiatives with people and groups.
- To promote physical health and wellbeing through health checks/screen and monitoring.
- Supervision of administration of prescribed medication and assessment of the service user for desired and non-desired effects if necessary to deliver prescribed medication.
- To maintain good clinical records and to complete reports when necessary.
- To provide and make available specialist aid and guidance to other professionals, agencies and carers.

- Service User Support
