True Care 247 are seeking caring and reliable Health Care Support Workers to join our growing Children & Young People Support Team, providing domiciliary care and outreach support services around the Warrington area. This is a position to support children and young people with additional needs, impairments, autism, behavioural issues and complex care needs in their homes and community environments. About this role

You will give person centred assistance and outreach help to each child or young person in accordance with their specific requirements and care plan. Support may comprise:

  • Assistance with everyday living and personal care
  • Social life and community access
  • Emotional and behaviour support
  • Supporting independence and confidence building
  • Enhancing wellbeing, safety and positive results
  • Working in partnership with families and professionals

Duties

  • Provide high quality care and assistance to children and young people
  • Support people in the community and home
  • Follow care plans, risk assessments and safeguarding procedures
  • Keep daily records and reports accurate
  • Promote dignity, independence and inclusion
  • Build positive working ties with families and professionals
  • Attend training and supervision meetings and team meetings

Prerequisites

  • Experience within care, support work, children’s services, SEN or community care
  • Attentive, patient and professional attitude
  • Good communication and team work skills
  • Knowledge of safeguarding and child protection
  • Flexible and reliable way of working
  • NVQ Level 2 or 3 in Health and Social Care
  • Experience supporting Autism, ADHD or learning impairments
  • Full UK Driving Licence
  • Medication, First Aid or Behaviour Support Training
