Responsibilities:- Help clients with daily chores including personal care, meal prep and medication reminders.
- Assist clients to preserve independence in their homes or care surroundings.
- Drive customers to appointments, social activities or errands in business vehicles or personal transportation if appropriate.
- Develop and implement individualised care plans to meet each individual’s needs.
- Monitor and report on clients’ health state and wellness, ensuring that concerns are reported properly to necessary professionals.
- Keep accurate records of care provided and any changes in the client’s condition.
- Improve the quality of life of customers through promoting social engagement and emotional support.
- Follow health and safety procedures, including infection control methods.
- Experience of working in a care home or providing home care is ideal but not essential; training will be given if appropriate.
- Ability to establish and implement individualised care plans efficiently.
- Excellent communication abilities (verbal and written) in English to comfortably liaise with clients and team members.
- Competent in the use of IT systems for record keeping and reporting.
- Excellent interpersonal skills to demonstrate empathy, patience and understanding of clients’ needs.
- Excellent organisational skills to successfully co-ordinate schedules and prioritise activities effectively.
