We are looking to appoint a Social Worker to our Adult Community Mental Health Services Team in North Hertfordshire. • Experience in working in a community context.
• Provision of service to a designated caseload of clients living in the Community who are experiencing, or recovering from, mental health challenges.
• To provide direct assistance and advice to Service Users and Carers within the recovery framework.
• To operate within a multi-disciplinary Team, in cooperation with a range of stakeholders within and outside of the Trust.
• Providing high quality social care services to people of all ages facing mental ill-health.
• The primary responsibilities of the post will involve assessing, developing care plans, and coordinating care from a social viewpoint for individuals utilising the program.
• Deliver recovery focused care and support to service users and their carers that is informed by the concepts of choice, control and independence as well as safety.
• You will have a caseload of people with a broad spectrum of mental health and social care needs.
• You will ensure that you are always reflecting on and developing your practice and will ensure your continuous professional development to meet the requirements set out by the professional regulator, Social Work England.
• If you are not already an AMHP you will be prepared to pursue the training needed to undertake the AMHP function as your career evolves.
• If you are an existing AMHP you will be on the Trust’s countywide AMHP rota.

• Champion social care, having a good awareness of legislation and statutory advice.
• To be the lead in the promotion of adult safeguarding practice, doing safeguarding enquiries and contributing to the development of care plans to protect adults and children from abuse.
• To undertake full assessments and reviews for people who may present with a high risk, or those with significant interpersonal, social, legal and/or environmental complexities.
• To ensure people are assessed appropriately against social care eligibility criteria as defined by The Care Act 2014.
• To provide information and advice to service users and carers and to colleagues to ensure that those who do not fulfil the criteria for social care eligibility are effectively signposted and supported.
• To ensure that appropriate health and social care services are commissioned with and for service users (including the obligation to provide direct payments) and that arrangements are in place for the local authority to invoice service users for the social care services provided, where appropriate.
• Regular risk assessments must be carried out to protect the safety of vulnerable people.
• To carry out assessments of carers including the identification of contingency plans and the provision of support.
• Experience in working in partnership to discover, develop and facilitate a wide range of recovery focused community services and to ensure that colleagues within the locality are also aware of advancements and best practice.
• To involve service users and their carers so as to enable the Trust to learn from their experiences and make improvements to service delivery.
• To maintain up to date electronic records of every work carried out.
• To do your own post qualifying and any necessary training to guarantee high standards of practice and continuance of your registration with the Social Work England.
• To offer management and reflective supervision to freshly qualified social workers and to unregistered social care staff.
• Supporting freshly qualified social workers through the Assessed and Supported Year in Employment (ASYE) including possibilities to learn as an ASYE assessor.
• To be willing to conduct duties as an Approved Mental Health Professional (AMHP) or to pursue AMHP training.
• To do your own regular supervision and to regularly reflect on your own practice as a social worker and AMHP.
• To conduct various tasks and duties which form part of the delegated social care responsibilities assumed by the Trust on behalf of Hertfordshire County Council.
• Follow Trust policies and procedures.
• To complete any more training needed.
• Perform any other duties as may be reasonably determined by your Line Manager.
