To provide a high standard of care and practice at all times, taking direction from Community Service Team Leader and respite care coordinator. To work as part of a multi-disciplinary team respecting the skills, expertise and contributions of others in the provision of holistic care and support to babies, children, young people and their families accessing the service. To treat each child and young person as an individual respecting their dignity and providing care that is sensitive to their needs. To undertake clinical duties both within the community setting and also within the Hospice.

Main Duties of the Job

  • To work under guidance and supervision ensuring the provision of high quality care and practice; in accordance with Derian House Children’s Hospice policies, standard operating procedures and guidelines for designated children or young people.
  • To accurately monitor and document the care for the children and young people and ensure that the care provided adheres to individual care plans.
  • Ensure that the children and young people’s daily routines and individualised needs are respected at all times.
  • To be responsible for monitoring, reporting and recording any changes in the condition of the children or young people being cared for, including health, emotional and social care needs.
  • To be responsible for administering medicines and pharmaceutical products to children undertaking respite visits, supporting symptom management and memory-making amongst many other tasks, but also working in-house in our hospice in Chorley.

It is key that you know if you apply for this role you will most of the time likely to be working autonomously, supported by a multi-disciplinary team to make decisions to best support families. As this is a role where you will be supporting families outside of the hospice, there will be many occasions where you are working alone, and need to be comfortable with this, with our systems and processes in place to make sure you are safe.

We are investing more in community and rotational roles that work across our in-hospice and community services, giving you the chance to build breadth of experience in a genuinely varied role. We also welcome applications from nurses with non-traditional routes into practice, including those with learning disability qualifications and those from non-paediatric backgrounds who have the right transferable experience.

We offer 35 days holidays per year, including bank holidays and increasing with length of service. Continuation of NHS Pension scheme (if eligible) or option to join Derian House Contributory Pension Scheme. Payment of professional NMC registration fees. Retail discounts through Blue Light card and Benefits platform. Free onsite parking. Subsidised meals in our diner. Flexible working in terms of part-time opportunities. Hours: 37.5 hours per week (full-time). Weekend, nights and evening work will be required. Part-time hours will also be considered.

Selection and Assessment Process:

Depending on the volume of applications there may be an initial preliminary 20-minute discussion, held virtually (MS Teams), to help sift candidates we wish to progress. The formal process for those invited, will be an onsite face-to-face assessment and interview.

Person Specification

  • Professional/Qualifications: Evidence of ability to work through competencies in all core clinical skills. Ability to record information in a concise, accurate and legible manner. Relate well with children, families and other healthcare professionals. Proven ability in the delivery of holistic care.
  • Personal Qualities: Ability to adopt a flexible approach to working, able to cover a range of shifts over a 7-day period. Honest and trustworthy. Ability to be supportive of colleagues, children and families. Reliable. Ability to work using own initiative. Proven ability to deal with complex and difficult emotional situations. Recognition of own limitations.
