To work as part of a multi disciplinary team delivering palliative and end of life care to Children and Young People (CYP) within the home environment, respecting the skills, expertise and contributions of others in the provision of holistic care and support to CYP and their families using the Derian at Home Service.

To be responsible for the assessment of the CYP’s care needs, including the development, implementation and evaluation of programmes of care.

To assist in the management and organisation of work as required by the Community Services Manager, ensuring effective communication is maintained at all times and to provide support for team members.

To play a key role in the supervision and assessment of designated staff as required

Main duties of the job

Key Accountabilities

Staff responsibilities include staff within the remit of the Derian at Home team, as specified by the Team Leader.

Support and assist the team leader in effective care delivery.

Main Duties and Responsibilities:

Managerial

To support the team leader in the day to day operational management of the service by providing leadership and supervision for clinical support workers and students.

To participate in the orientation of new staff, students and volunteers. Acting as practice assessor as and when required.

Clinical

To act as a positive role model by using evidence/research based care and practice.

To be proactive in identifying ways of improving practice. Contributing positively to the development and audit of standards.

Provide and oversee the delivery of high quality, clinically effective and evidenced based nursing care and practice in accordance with Derian House policies, standard operating procedures and guidelines.

Ensure that care for children and young people is assessed, planned, delivered and evaluated respecting children and young people’s daily routines as far as is possible.

To be responsible for providing safe and effective care in line with the hospice’s operating procedures and guidelines and act as a resource for all members of the multi disciplinary team providing specialist professional, care and support.

To adapt the delivery of care according to the emotional, physical and environmental demands.

To be competent in Derian House’s core and clinical competencies and individual care needs.

To be responsible and accountable for the safe administration of medicines within the home ensuring medicine management policies and procedures are adhered to as per NMC guidelines.

To provide 24 hour on call service for telephone advice and home assessment visits using advanced skills to base decisions on.

Clinical/Quality Assurance

Ensure the respect, dignity and rights of children, young people and their families are upheld at all times.

To have the opportunity to undertake training as a supplementary and independent prescriber adhering to NMC Guidelines, if requested by the organisation.

To be responsible and accountable for the safe administration of medicines within the community, ensuring Medicine Management Policies & Procedures are adhered to.

To take an active role in the pursuit and promotion of quality through audit and research and participating in the hospice governance programme.

To support CYP and families at the end of life, crisis management and bereavement care.

Communication

Regularly attend team and other meetings as appropriate, contributing positively to discussions and debates and participating in shared decision making.

Ensure effective communication including the dissemination of information accurately and timely to support effective team working.

Work collaboratively within multi disciplinary/multi agency teams both internally and externally to Derian House through the establishment of effective communication systems to support the delivery of high quality care.

To promote the work of Derian House be delivering external presentations and training as requested.

Key Working Relationships

Key working relationships include outside agencies from health, education and social care involved with the care and support of children/young people and their families.

No direct budgetary responsibility.

Foster strong links with referring agents e.g. district general hospitals, tertiary centres and community teams.

Professional

Act at all times in accordance with relevant legal, ethical and professional legislation and frameworks (i.e. Children Act, NMC Code of Professional Conduct).

Ensure that personal actions and conduct comply with hospice policies and procedures as well as the organisational values.

To maintain confidentiality in all aspects of the role, notably information regarding the children and their families, staff and the organisation itself. Disclosure to any unauthorized person is considered a disciplinary offence.

Maintain relevant, accurate and contemporaneous records and reports, respecting and maintaining confidentiality at all times in accordance with NMC Guidelines, Derian House Children’s Hospices policies & procedures and the Data Protection Act (1998).

Health and Safety:

To report and record all accidents and incidents in line with hospice procedure safeguarding the welfare and safety during working hours with regard to the Health and Safety at Work Act.

Ensure health and safety requirements in relation to the provision of care to children, young people and families are managed correctly through the provision of appropriate risk assessments.

To be able to undertake the mental, physical and emotional demands of the role, whilst at the same time taking care to safeguard their own health and safety as well as fellow care team members, children, young people and their family.

Adhere to the prevention and control of infections code of practice (Health & Social care Act 2008) and Derian House Children’s Hospices policies and procedures to prevent and control infection.

To adhere to the Derian House lone working policy.

Hours:37.5 hours per week (full-time). Weekend, nights and evening work will be required. Part-time hours will also be considered.

Professional/ Qualifications & Special Training & Education

  • RSCN, RN (Child) or RGN with a proven track record of paediatric competency.
  • Evidence of continued professional development.
  • Advanced clinical assessment skills
  • V300 Independent prescriber
  • Advanced/Enhanced Communication Skills Training
  • Experience of working in a relevant paediatric environment.
  • Experience of working in a multi-disciplinary Team.
  • Proven ability of delivering care to children with complex health needs and palliative/end of life care needs.
  • Involvement in clinical audit with a knowledge of risk assessment sand clinical governance
  • Proven ability of working within a community setting.
  • Knowledge of key recent developments in palliative care.
