This is an exciting and unique opportunity for an experienced GP looking to become a service lead for a new and innovative service.
This role is advertised as full time; however, we are happy to consider part-time applications.
The National Ageing Well programme aims to build upon the progress already made with partners on service provision at the interface between health and social care, focusing on the prevention of avoidable hospital and residential care admissions and pro-active care provision linked into primary care network services.
Team Up Derbyshire is the local approach. The plan is to create one team across Health and Social Care who see all housebound and complex patients in a neighbourhood. It means one team to do it all: urgent, planned, or anticipatory care and support.
Derby City South PCN will be delivering this programme on behalf of the patients within the PCN and in conjunction with community partners and services within the PCN boundary.
This is not a new or add-on to existing services but a teaming up of existing services which incorporates all the requirements of the national Ageing Well Programme.
The local ambition is greater than the national programme; we are aiming not only to transform anticipatory care, community urgent response, and support to care homes but also GP acute home visiting.

- Work in conjunction with the PCN Service Operational Lead to plan, develop, and deliver safe, effective, personalised, and integrated Primary Care to people who need services in their own home, even if that is a care home, for the community covered by their service.
- Provide strong clinical leadership to ensure the development of a high-quality service focused on value and outcomes for the population they serve through a culture of continuous improvement and learning.
- Be responsible for clinical governance and quality assurance ensuring excellent patient care and compliance with all associated national and local requirements.
- Work with clinical leaders and external partners, communicating across the system to ensure that they work together to optimise the outcomes for their populations and minimise health inequalities.

Qualifications
- A vocationally trained and accredited GP
- MRCGP
- Current registration with GMC
- On the GP performers list

- Recognised qualification in Care of the Elderly

Experience
- Experience and evidence of an interest in care of the Elderly
