Matlock and Ashover Practice is made up of two surgeries one at Imperial Road Surgery in Matlock with 7600 patients and a second Surgery in the village of Ashover with 2400 patients. We have one patient list allowing our patients to be seen at either practice. We have five GP partners and a further five salaried GP\’s we are also a training practice with both registrars and medical students. Our primary aim is to serve the General Public with first class medical services.

We are looking to recruit a community matron to ideally work over three to four days per week up to a maximum of 30 hours per week.

You will work with patients to promote shared decision making regarding their care plans, encourage them to make informed decisions about their health and to self- manage long-term conditions with the aim to prevent hospital admissions.

Person Specification

Qualifications/Training

  • First Level relevant Degree or Equivalent
  • Non-Medical Prescriber
  • Evidence of master\’s level study/commitment to study.
  • Master\’s Level Degree

Experience

  • Demonstrates a high level of relevant clinical experience.
  • Experience of working with people with complex health needs
  • Able to carry out physical examination and comprehensive health assessment.
  • Demonstrate competence in safe medicines prescribing.
  • Excellent level of clinical competencies
  • Evidence of ability to direct and coordinate programmes of care working autonomously and collaboratively.
  • Ability to appraise complex situations and facts and take appropriate action.
  • Evidence of knowledge and experience of case management
  • Experience of managing patients with long term conditions
  • Demonstrate a high level of clinical assessment skills
  • Experience of working with older, palliative care, chronic disease
  • Current evidence-based skills of older people\’s issues
  • Continuing professional knowledge and development
  • Experience of multi-professional working
  • Experience of teaching and mentorship
  • Experience of audit and research

  • Due to the requirement to undertake visits the post holder is required to be a car driver with a valid driving licence.
