Be part of a multi-disciplinary team. Deliver short-term, holistic assessments. Advocate for people’s social care needs. Ensure the person’s voice is central to their care. Work closely with Derbyshire Adult Care & Living Well partners.

Living Well Derbyshire offers accessible mental health support through a mix of healthcare professionals and voluntary sector partners. We focus on independence, wellbeing, and strengths-based care. You’ll work with people who have:
- Primary mental health needs
- Learning disabilities
- Drug/alcohol dependencies
- Physical health needs
