We are looking to recruit a full-time Senior Practitioner for the Adult Care Assessment and Triage Team. The team is made up of Social Workers, Social Care Practitioners and Occupational Therapists. The Senior Practitioner will provide countywide leadership to the Adult Care Assessment and Triage Team who provide professional assessment and triage at the front door of Adult Care. The successful applicant will be able to demonstrate the skills, knowledge, and experience to undertake this role to provide a safe, accountable, cost effective and efficient service to help people using our service achieve their outcomes.

The Senior Practitioner will have the ability to support the Service Manager with establishing and maintaining close relationships with partner agencies and support the development of our offer to Derbyshire residents. You will use your relationships and skills in supporting efficient and creative solutions to resolve problems. Applicants should be able to demonstrate an excellent track record in their ability to support others and develop professional autonomy and accountability.

We are looking for applicants who are motivated, enthusiastic and who embraces ongoing improvement and development. You will promote a strength-based approach and be able to use your influence to encourage a shared commitment to supporting people to maintain independence, choice and control over their own lives.

The post is for a 5 from 7 working pattern but at the present time staff are expected to work on a Monday to Friday basis. Prior notice will be given in advance if this changes.
