Derbyshire County Council are inviting applications for 2 qualified social workers to work in our established Community Hospital and Community Support Bed Assessment and Discharge Team at our Community Support Beds and Community Hospitals.

Main duties of the role:

  • Work from community support bed or community hospital sites, primarily Whitworth community hospital, Clay Cross and Staveley Centre.
  • Contribute to goal setting and discharge planning, complete Care Act assessments and support plans, support people within the Mental Capacity Act and make safeguarding personal.
  • Travel to other sites as required and adopt hybrid working to meet service needs.

What we expect from applicants:

  • Person centred and strength-based assessments and timely discharge planning support.
  • Confident in using the Care Act 2014, Making Safeguarding Personal and the Mental Capacity Act.
  • Team player who works closely with other disciplines and teams and can work autonomously with a positive and can-do attitude.
  • Ability to work unsocial hours, including weekends and Bank Holidays, with appropriate pay rates.

Qualifications and experience:

  • Qualified social worker with experience supporting people in community hospital and bed setting.
  • Knowledge of the Care Act 2014, Safeguarding Personal, Mental Capacity Act and NHS community services.

Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and requires a DBS check.
