We are looking for qualified nurses to join our Ashbourne District Nursing Team to complement our current team. The position in this team is for a part-time employee of 30 hours a week based at ST Oswald's Hospital. Due to the geography and location of Ashbourne, there is an element of Rapid Nursing within this position where UCR response rates must be met. This is a great opportunity to join one of our teams in the Derbyshire Dales.
Main duties of the job:
- Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
- Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
- Provide advice and support to junior staff in the delivery of high-quality care and actively support the Community Nursing sister/charge nurses in the management of the health care environment.
- Implement / refer on to the relevant services as appropriate.
- Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

- NMC registered Nurse (including newly-qualified and soon-to-be-qualified)
- Evidence of recent relevant training
Experience and Knowledge Required:
