Are you an experienced Registered Nurse, seeking a fabulous opportunity to work within the Integrated Community Teams in Chesterfield and North East Derbyshire? Come and join the Self-Management Facilitators! Our Self-Management Team empowers patients and their support networks to take control of their own nursing needs wherever possible. With an exceptional success rate, we focus on promoting independence, building confidence, and enabling individuals to manage their care safely and effectively. We work collaboratively with patients to help them develop the skills and knowledge required to take responsibility for their own health needs—supporting better outcomes and improving quality of life.

Previous community experience is essential but we offer a robust local induction tailored to individual needs.

Main duties of the job

The successful applicant will work under the support and guidance of the District Nurse to facilitate patient self-care and promote independence within their own home environment. Your responsibilities will include assessing, planning, implementing, and evaluating individualised patient care in a compassionate, safe, and clinically effective manner. Throughout all aspects of your work, you will maintain the highest standards of patient privacy, dignity, and respect. A key requirement of this role is the ability to travel daily across both the Chesterfield and North East Derbyshire localities to visit patients in their own homes.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire. We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.
