We care about our community; we care about our people. We have a rare and exciting opportunity for a self‑motivated and passionate individual to join our team in the role of Adult Health Team Leader. The role of the Health Team Leader will be to lead and manage the Adult Health offer within the Trust, which will be delivered across Derby City and Derbyshire.
In this role, you will manage a small team to deliver a high-quality adult health service for adults experiencing health inequalities for a range of reasons. The service will be community-based and will offer both holistic and targeted support.
The postholder will support the Adult Health Manager in the ongoing development of the Adult Health offer, ensuring programmes remain responsive to local need, emerging health priorities and developments within the wider health and wellbeing landscape. They will contribute to the growth and continuous improvement of services through effective partnership working, community engagement, programme development and the identification of new opportunities to improve health outcomes for local residents.
