We are seeking to appoint a passionate Shared Lives Co-ordinator to be part of the Derby City Shared Lives Scheme.
Derby Shared Lives is a Local Authority run Scheme that supports a number of vulnerable individuals with differing needs by matching them to carers, who in turn deliver strengths based, person-centred support against identified outcomes either within their own Home or in the community with the support of our Co-ordinators.
You will need to be a highly motivated individual who can support carers to deliver the best possible care and support to our customers, working within the requirements of the Care Quality Commission (CQC).
You will need to be an excellent communicator, have good organisational skills with the ability to support and advise carers to manage risks and promote successful outcomes for our customers within the Scheme. You will need an understanding of Adult Safeguarding procedures and the ability to recognise and implement these within your role.
You will also be expected to process carer invoices for payment, monitor and maintain the carer training database and track progress whilst facilitating continuous development.
As part of your role, you will support carers to track customer progress against their defined outcomes using an outcome tool.
You will be expected to visit Shared Lives carers in their homes and obtain feedback from customers using the Service. Experience of working with a wide range of customer groups would be an advantage.
You should have the ability to liaise with external and internal stakeholders at all levels, promoting collaborative working and effective communication.
