Hours: 23 hours a week - Monday – Thursday: pm – pm. Friday: 12 noon –

We are seeking to appoint a passionate Shared Lives Coordinator to be part of the Shared Lives Scheme. We want the best for our vulnerable adults, and we want to ensure the best provision to our valued carers who are delivering a unique service. We are looking for like-minded people to join the team in Derby and be champions of outstanding care and support.

You will need to be a highly motivated individual who can support carers to deliver the best possible care to our customers. You will need to work with Shared Lives carers to ensure that the Scheme delivers a person-centred approach to our customers within the requirements of the Care Quality Commission (CQC). You will also need to be an excellent communicator, have good organisational skills with the ability to support and advise carers to manage risks and promote successful outcomes for the customers within the Scheme.

You will need an understanding of Adult Safeguarding procedures and the ability to recognise and implement these in your role. You will be expected to visit Shared Lives Carers in their homes and obtain feedback from customers using the Service. Experience of working with a wide range of customer groups would be an advantage.
