About Derby Aboriginal Health Service

Derby Aboriginal Health Service (DAHS) is an Aboriginal Community Controlled Health Service (ACCHS) dedicated to providing holistic primary health care services to the Aboriginal and Torres Strait Islander peoples in Derby and surrounding areas. We offer a variety of innovative public health programs alongside direct comprehensive care, ensuring compliance with various government and private health agreements.

Position Overview

The NDIS Community Connector Co-Ordinator plays a crucial role in assisting individuals with disabilities to determine their eligibility for the National Disability Insurance Scheme (NDIS) and guiding them through the access process. This position involves gathering necessary information and evidence for eligibility, coordinating planning meetings, and connecting individuals with alternative services when access is not granted.

Key Responsibilities

  • Act as the primary liaison and support for families utilizing our services.
  • Build and maintain strong relationships with individuals with disabilities, their families, and service providers.
  • Ensure cultural safety is integral to service delivery.
  • Document and update client files accurately to reflect service activities.
  • Facilitate access processes by gathering required documentation and coordinating meetings with stakeholders.
  • Develop local connections with DAHS, schools, and community-based organizations.
  • Contribute to policy formulation and process improvements within DAHS.
  • Engage in continuous quality improvement activities and training.

Candidate Profile

The ideal candidate will possess a strong background in social services, particularly working with families facing complex needs. They will demonstrate effective communication skills and the ability to engage with marginalized communities.

  • Experience in health or social service delivery, particularly with Aboriginal communities.
  • Ability to collaborate with government and non-government agencies.
  • Proficiency in using technology and maintaining detailed records.
  • Commitment to cultural sensitivity and understanding of local Aboriginal issues.
  • Valid National Police clearance, WA Class C driver’s license, and working with children check.
  • Willingness to undertake NDIS Worker Screening Check and hold a First Aid or CPR Certificate.
