The Advanced Nurse Practitioner (ANP) has been assessed as competent in practice using their expert clinical knowledge and skills. They have the freedom and authority to act and make autonomous decisions in the assessment, diagnosis and treatment of patients. The ANP provides an opportunity for patients to receive timely care and negate unnecessary delay in receiving treatment without the need to refer to a GP. ANPs in primary care are able to develop close, long-term relationships with their patients and work in partnership with them to achieve optimum health. They are autonomous in making decisions based on assessment, diagnosis and interpretation of test results.
Main duties of the job
- Working collaboratively with the General Practitioners and the wider Primary Health Care Team to support the clinical and wellbeing needs of the patient.
- Safe and competent clinical decision-making skills must be demonstrable including assessment, diagnosis, treatment and evaluation of care for patients presenting with minor illness within a Primary Care setting.
- Provide high quality nursing care from the patient's first contact through to resolution or referral as appropriate, in accordance with standards of good practice.
- To administer and prescribe medication accurately and safely according to established organisational policies and directions, and in accordance with NMC guidelines.
Job responsibilities
- Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
- Implement and evaluate individual specialised treatment plans for chronic disease patients.
- Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patient's health.
- Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF.
- Provide wound care.
- Provide specialist clinics such as contraception.
- Undertake the collection of pathological specimens.
Person Specification
- Qualifications: RGN with current NMC pin or Emergency Care Practitioner Independent/Supplementary Prescribing MSc/Post graduate Diploma: Nurse Practitioner in Primary Care or equivalent.
- Knowledge: Knowledge of health promotion strategies, awareness of local and national health policy, knowledge of clinical governance issues in primary care, knowledge of patient group, knowledge of public health issues, able to identify determinants on health in the local area.
- Experience: Employment within a Primary or Urgent Care setting, experience of Chronic Diseases.
