The post holder will be responsible for directly supporting the Hospice at Home team in Gateshead with the administration and coordination of services and schedules. They will provide administrative support to the team which provides domiciliary care to patients at the end of life who wish to be cared for at home. The post holder will also work with the End of Life care facilitator to support administration required for Palliative and End of Life care education across the trust.

Main duties

  • Support and assist the Nurse Coordinator to deliver all day-to-day activities of the team, including planning home visits for a team of Band 3 support workers.
  • Develop and deliver high-quality administration processes to aid safe and effective clinical services in a community setting.
  • Respond to external and internal telephone enquiries and take appropriate action.
  • Prioritise telephone calls regarding patient information and potential interventions, ensuring timely handling.
  • Respond to and manage e-mail queries and requests for meetings on behalf of the team.
  • Maintain the diary of the Nurse Coordinator and other designated team members; schedule appointments and resolve conflicts.
  • Prepare agendas, collate papers and distribute documents to meeting attendees as directed.
  • Deal with and collate data; contribute to preparation and development of multiple reports of varying content.
  • Prepare presentations, letters and other documents to a high standard.
  • Support End of Life care facilitator with education administration.

Job Responsibilities

Daily use of Emis Community module. Maintain confidentiality and work with limited supervision using own initiative. Be aware of team priorities and assist in meeting deadlines. Data analysis and interpretation as required; monitor progress against targets; manage electronic and hard copy filing and bring-forward systems.

Person Specification

Essential

  • Qualifications: Good general education (minimum 4 GCSEs including Maths and English, or equivalent); NVQ level 3 in Business Administration or equivalent.
  • Knowledge: Computer packages (Word, Excel, Access, Outlook, PowerPoint); knowledge of Data Protection and Caldicott Principles.
  • Experience: Supporting managers or teams of staff; working with computer systems/databases; experience in community-based healthcare services desirable.
  • Skills: Effective verbal and written communication; ability to prioritise workload.

Desirable

  • Experience of NHS environment; experience in palliative care.
  • Knowledge of Healthcare IT systems.
