Denbighshire Community Support Services recognise that our most important resource is our staff. We are looking to recruit a Social Care Practitioner to the Complex Disability Team which is based in Denbigh. The post holder will be working as part of a multidisciplinary team aiming to enable individuals to optimise their level of independence within their own homes and communities.
